What is the intake form & what is it being used for? Intake form was created to:

• Gather information about who you are and what you are looking for in a shidduch.
• Give the shadchanim in our office the ability to work together as a team to find you a shidduch.

Complete the intake forms to the best of your ability. If you are unsure about an answer, then select the answer that seems most appropriate and continue with the form. Let us know in your interview that you would like us to review your response.

What the form will not do:

If you simply fill out the in-take form as instructed, then your information will only be available to the shadchanim in our offices. Our global database of over 100 Shadchanim will NOT have access to your information, as this form is only an intake form for the shadchanim in our offices.
